2026 PCORI Fee Amount
- The PCORI fee is paid by issuers of specified health insurance policies (for fully-insured plans) and plan sponsors of applicable self-insured health plans (such as employers with self-funded plans).
- For the period from October 1, 2025, to before October 1, 2026, the fee is $3.84 per covered life, as per sections 4375 and 4376, IRS Notice 2025-61
- Due Date: July 31, 2026, for plan years ending in 2025.
Who Pays the PCORI Fee?
Issuers of Fully Insured Health Plans
Health insurance carriers that issue fully insured group plans are responsible for paying the PCORI fee. The insurer files and pays the fee on behalf of covered lives under the policy.
Sponsors of Self-Insured Health Plans
Employers or plan sponsors that fund their own self-insured group health plans must calculate, report, and pay the PCORI fee directly to the IRS.
Employers with Self-Funded HRAs
Health Reimbursement Arrangements (HRAs) treated as self-insured plans are subject to the PCORI fee, and the employer/plan sponsor must pay it.
Sponsors of Retiree-Only and COBRA Plans
Self-insured retiree medical plans and COBRA continuation coverage count as self-insured plans, so their sponsors are responsible for the PCORI fee.
Employers with Level-Funded or Integrated Plans
If an employer has multiple self-insured arrangements with the same plan year and sponsor, they may combine coverage for one PCORI fee payment rather than multiple filings.
Common Challenges in PCORI Filing
Manual Data Collection for Covered Lives
You must gather enrollment counts and supporting reports manually from multiple sources. This increases effort and makes it easier to miss data or use inconsistent numbers.
Complex Covered-Life Method Selection and Documentation
Choosing the right method (Actual Count, Snapshot, or Form 5500) can be confusing without guidance. Manual filing also requires you to keep clear backup to justify the method used.
Higher Risk of Form Entry Errors on Form 720
Manual entry on Form 720 and the PCORI line increases the chance of typos and wrong amounts. There’s no auto-calc support, so mistakes can carry through to the final total.
Paper-Based Record Management
Keeping copies of calculations, plan documents, and payment confirmation manually is harder to manage. Records can be misplaced, making audits and future reference difficult.
Payment Coordination and Proof Tracking
Manual filing often means extra steps to ensure payment is made correctly and matched to the filing. Tracking payment confirmation and keeping proof organized becomes an additional burden.
